    Job Description

    The Behavioral Health Case Manager serves as a member of a multi-disciplinary patient care team for NATIVE HEALTH’S Behavioral Health Division. The Behavioral Health Case Manager provides case management services that support the treatment goals of the patients care plan. The Behavioral Health Case Manager represents the company professionally and positively to enhance and promote the core values and mission of NATIVE HEALTH, always exercising utmost discretion, diplomacy and tact in customer transactions.

    KEY ACCOUNTABILITIES :

    1. Provides non-intensive case management services in support of Behavioral Health patients and providers.
    2. Supports the behavioral health program and its team members.
    3. Participates in quality improvement and quality assurance activities for the department.
    4. Supports the organization as needed

    Qualifications

    QUALIFICATIONS/EXPERIENCE/REQUIREMENTS :

    1. Bachelor’s Degree in Social Work or a related field is required.
    2. Master’s Degree in Social Work or related field is preferred.
    3. Two years of related case management experience is required. Experience working with underserved and diverse populations is preferred.

    Standard Employment Requirements

    1. Must possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver’s license during employment. Must be insurable under NATIVE HEALTH’s liability auto policy.
    2. Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
    3. Must pass substance abuse testing upon employment and submit to random testing during employment.
    4. Must provide an updated immunization (IZ) record at the time of employment.
    5. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
    6. Must obtain a First Aid/CPR Card within (90) calendar days of employment and maintain a valid CPR Card during employment.

    4. Positive attitude and the ability to advocate on behalf of the patient’s needs.
    5. Knowledge of existing social and behavioral health resources for patient care.
    6. Excellent written and verbal communication skills and ability to establish rapport.
    7. Good documentation skills, including detailed and accurate documentation.
    8. Mental health/substance abuse terminology knowledge and understanding. Knowledge of mental health issues of the populations served.
    9. Ability to work independently with strong sense of focus, task-oriented, non-judgmental, and clear sense of boundaries.
    10. A strong sense of and respect for confidentiality.
    11. Courteous, honest and professional at all times.
    12. Customer service oriented.
    13. Ability to plan own work and carry out assignments effectively.
    14. Excellent computer skills with Microsoft Office and electronic health record.
    15. Remain current and complete on time, all assigned Relias trainings.
