    Intake Coordinator / Community Outreach Liaison

    Community Psychiatric Institute, a leader in Behavioral Healthcare and Chemical Dependency treatment has an opening for a Full-time Intake Coordinator for our Partial Care and Outpatient adult treatment programs.

    We are looking for a dedicated and compassionate mental health professional to join our team.

    HOURS : Full-time

    MINIMAL QUALIFICATIONS Required :

    Master’s Degree in Behavioral Health or related field of study.

    Experience with Substance Abuse & Co-occurring clients

    Experience performing intakes and assessments.

    2+ years experience working with co-occurring populations.

    Strong communication and writing skills.

    JOB DESCRIPTION :

    The Intake Coordinator will be responsible for facilitating patient intakes and admissions into our Partial Care, IOP and Outpatient programs. The Intake Coordinator will assist with prior authorization submissions for new and existing clients.

    Duties & Responsibilities:

    The coordinator will be the point of contact for Hospital discharges and referrals and facilitate the intake process for all patients.

    Perform Intakes and Assessments for mentally ill, substance abuse and co-occurring consumers as required by State regulations.

    Complete NJSAMS as needed.

    Contact Medicaid MCO's for Initial Prior Authorizations.

    Facilitate collaborations with community agencies and hospitals promoting our treatment program.

    Serve as liaison with health care professionals to facilitate agency referrals (i.e. discharge planners, social workers, residential managers and agency employees)

    Screening patient clinical records for appropriateness of treatment and working with our nursing staff for medical clearance when appropriate.

    Other Job duties include:

    Coordinate with referral sources to promote census growth and development.

    Develop working relationships with social workers/case managers at hospitals, residential facilities and community agencies.

    Assist with the processing of facility admissions

    Assist in verifying client insurance eligibility.
