Phix Health is looking for an energetic MSN or BSN prepared RN who would like to complement our state of the art Chronic Care Primary Care organization. Our clinical specialty focuses on providing an integrated holistic care process to our clients that includes recognizing their emotional and mental health needs.

This is a unique opportunity for an RN to work independently in an educational and consulting capacity.

We are looking for someone who embraces helping clients as they struggle with the emotional and mental health challenges confronted by the changes in their personal health journey.

This person will be responsible for creating and implementing a plan of care to meet the unique needs of clients who would benefit from one on one counseling/educational services including: hospice, advance care planning, depression, adjustment disorder, anxiety, and more.

As a critical part of the care team, this person will work directly with the client, primary care physician, psychiatric consultant, and hands on care providers to provide behavioral health integration into their plan of care.

Training for counseling, behavioral health and screening interventions will be provided.

Special skills desired include:

Strong communication skills demonstrating the ability to collaborate effectively in a team setting.

Able to effectively engage patients in a therapeutic relationship

Understands the importance of creating, maintaining, and assessing measurable care goals.

Comfortable with providing inpatient face to face care interventions and facilitating outside referrals to evidence based care.

Provides important input into case conferences with care team members.

This is a part time position with the possibility of growth into a full time position in the longer term.
