Job Description
Requirements: * Must be at least 18 years of age or older * Must have completed two years of full time study at an accredited college or university * An individual with a high school diploma or general equivalency diploma (GED) may substitute one year of full-time employment in a supervisory capacity, agency, or community-based agency for each required year of college * Must be able to speak both English and Spanish and must be able to communicate in both English and Spanish Skills, Knowledge, and Abilities: * Must possess analytical and observational skills * Must possess good people skills * Must possess good communication skills * Must be computer literate, knowledge and ability to use word-processing, spread sheets, and software systems * Must be able to visit various client's home throughout Cameron County, Hidalgo County, Willacy County, Starr County * Must be able to communicate well Texas Department of Health & Human Services and various Health Management Organizations Permits and Licenses: * Must have a valid Texas driver's license and must be insurable through the agency's insurance carrier Responsibilities and Duties * Shall prepare and complete all documentation for Initials and Transfer Start of Cares to include: assessments, practitioners statement, 2067s, 3040, all acknowledgements, admissions service agreement, coordination of care, and verification of tasks form * Shall hire attendants according to client's specific needs and as requested by client or family * Shall complete attendant applications * Shall conduct Supervisory Visits * Shall complete client Yearly's * Shall be responsible for the delivery of EVV devices
