Meet with newly assigned client and conduct introductory session.
Participate//facilitate clinical staff meetings as scheduled.
Participate in weekly and monthly individual and group clinical supervision meetings with clinical director.
Participate in clinical peer reviews as assigned at least monthly or quarterly as part of quality assurance.
Ensure all client referrals for services made on or offsite have follow up documentation in the record, releases of information obtained, etc…
Complete, sign, credential, and date all Suicide Risk Assessments upon admission.
Complete, sign, credential, and date all Trauma Assessments when completed.
Complete, sign, credential, and date all Spiritual Assessments when completed.
Complete, sign, credential, and date all Legal Assessments when completed.
Assist with AMA blockage onsite and at housing via telephone when possible.
Complete, sign, credential, and date all clinical progress notes within 48 hours of sessions.
Collaborate with Medical Director when necessary, to facilitate Baker Acts and or Marchman Acts when possible.
Participate in staff meetings-report to Management issues or concerns as needed, etc…
Complete required trainings within orientation period and annually.
Participate in clinical staff development-recommended trainings/competencies, workshops, and or in-services to enhance skills and or performance.
Ensure assigned client satisfaction surveys are completed at discharge or when a client transfers from one level to another.
Ensure client outcome surveys are completed for assigned clients.
Complete, sign, credential, and date all Individualized Treatment plans for all assigned clients.
Complete, sign, credential, and date all Treatment Plan Reviews.
Complete corrective actions when assigned.
Complete, sign, credential, and date all ASAM’s-Admission, Continued Stay, Transfer, Discharge.
Participate in Clinical Competencies at least annually.
Complete, sign, credential, and date all Transfer Summaries.
Complete, sign, credential, and date all Discharge Summaries.
Develop, Implement resources for clinical content for group sessions.
Ensure communication with Billing company/utilization reviewer to complete concurrent reviews, correct and or provide any record documentation needed for authorization of treatment days.
Ensure that an attempt is made to conduct family therapy weekly and documented.
Ensure that individual sessions are conducted weekly for all assigned clients.
In collaboration with Human Resource, provide proof of trainings/CEU’s obtained offsite.
Abide by HIPPA and Confidentiality Laws.
Collaborate with Clinical Director and Lead Behavioral Health Tech to approve all clinical passes for clients in community housing level of care.
Collaborate with Clinical Director and Lead Behavioral Health Tech to approve phone calls for clients in community housing level of care.
Conduct client records audits at least quarterly for content and required documents.
Maintain current census for all levels of care.
Ensure all progress notes are completed, signed, credentialed, and dated within 48 hours of sessions.
Maintain credentials active and provide Human Resources with current active licensure and certification.
Complete and Review incident reports when necessary.
Participate in an internal quality assurance method of assessing client’s status, quality of services, and aggregate data.
Complete corrective actions when assigned.
