    Join our growing Behavioral Health Medication Management Team.

    Working within your training and the established scope of practice assist the Behavioral Health Medication Management Team in delivering quality patient-centered care in an efficient manner.

    What you will do:

    Participate in daily huddle and pre-visit planning with care team to address care needs effectively.

    Rooms patients, measure vital signs, such as pulse, temperature, blood pressure, weight, and height.

    Engages patients regarding current symptoms, medications, allergies and other health related information using standardized screeners, and update EHR.

    Respond to patients requests in consultation with medical providers and RN.

    Clean and sterilizes rooms.

    Coordinate referrals including coordination with other departments and facilities to support follow-up appointments.

    Work with care teams and other staff in strategies for preventative care and improving patient health outcomes.

    Engage patients with their treatment and coordinates with other care team members to support whole-person care (e.g., RN, Care Coordinator, Behavioral Health Consultant, etc.)

    Gather relevant information about social drivers of health including social history, gender identity, to help deliver culturally informed care.

    Help assist with outside referrals as directed by providers.

    Maintain electronic medical records and documents all relevant patient information in the EHR.

    Assist patients needing prior authorizations.

    What you will need:

    To have completed a post-secondary Medical Assistant program or equivalent.

    A current certification or registration with an approved national examination agency; or the ability to become certified or registered within six months of hire.

    1-2 years of experience in an outpatient clinic setting.

    Current BLS certification or the ability to become certified within 60-days of hire.
