Salary: $20.00 Per/hour

As a Case Manager, you will be at the heart of transforming lives by helping clients facing persistent mental health and substance use challenges to live joyful and fulfilling lives. Key Responsibilities:
Provide life-changing care coordination for clients with persistent mental health and substance use challenges, both in the community and in the office.
Coordinate care with mental health and health care providers to create a seamless, integrated care experience.
Assist clients in navigating services such as legal aid, housing, employment, substance use treatment, medical care, and more, helping them overcome barriers to successful recovery.
Collaborate with your team to ensure each client receives the support they need to thrive.
Build partnerships within the community, making it easier for clients to access the resources they need.
Participate in trainings as needed.
Qualifications:
A BA/BS in Social Work or a related field.
At least 3 years of experience in care coordination.
Someone who is compassionate, an excellent communicator, and ready to make a difference.
The ability to travel, with a valid driver’s license and reliable vehicle.
