We are seeking a dedicated and experienced Medical Assistant to join our Chronic Care Management team, providing support to doctors and their patients. The successful candidate will play a vital role in delivering quality care to patients with chronic conditions, working closely with medical providers to ensure the well-being of our patients.
Collaborate with physicians to create care plans for patients with chronic conditions
Assist in patient education and self-management support
Coordinate follow-up appointments and ensure patients adhere to treatment plans
Monitor patient progress and communicate any concerns to the healthcare team
Document patient encounters and maintain accurate records in the electronic health record system
Conduct medication reconciliation and assist with prescription refills
Perform basic patient assessments and vital sign measurements
Provide empathetic and compassionate care to patients with chronic conditions
Completion of a medical assistant program (not required if you have experience in the medical field.
Proficiency in medical terminology and EMR systems, specially e-clinical works
Excellent communication and interpersonal skills
Team player and able to work with others to achieve department goals.
Ability to multitask in a fast-paced clinical environment
Compassionate and empathetic approach to patient care
Bilingual due to the high number of Spanish-speaking patients
The hourly salary ranges from $18.00 to $21.00, the hourly rate is commensurate with experience and certifications.
