    Summary of Position:

    Provides direct care to residents in the facility, primarily for their activities of daily living, plan of care, assistance with therapeutic activities program, restorative programs, and documentation as required by the facility.

    Qualifications:

    Certified Nurse Assistant with a current State of TENNESSEE license.

    CPR Certification

    Must be able to read regulations and patient records and write clearly.

    Must be able to communicate in a courteous, friendly manner.

    Physical requirements attached.

    Education and/or Experience:

    Minimum Nurse Assistant Certification. One year of experience working as a Certified Nurse Assistant including some medical/surgical experience and awareness of the needs of geriatric residents.

    High School graduate or GED preferred.

    Primary Duties and Responsibilities:

    Assistance may range from total to partial to stand by depending upon physical and cognitive ability of resident.

    Assist residents in and out of bed and chair or wheelchair with the correct mechanical lift. May require the assistance of another aide, depending on weight and physical abilities.

    Assignments will be rotated daily: includes bath aide, red napkin, hall trays, and hall.

    Assist residents in dressing or undressing. Fold or hang clean items and place soiled items in hamper (family laundry) or designated area (facility laundry).

    Assist residents in using bathroom and peri care. May be required to provide, remove and clean bedpans, urinals and bedside commodes.

    Take vital signs of residents, e.g. temperature, blood pressure, pulse, respiratory rate and record information.

    Assist residents to dining area for meals or provide meals in room, as resident prefers. May be required to assist in tray setup and feeding of residents, document intake of nourishment and assist as needed. Serve between meal nourishment. Record amount eaten. Except when contraindicated, maintain clean pitcher and cup, and fresh water and ice at bedside of each resident.

    Remove soiled bedding and replace with clean linens as necessary.

    Assist in personal grooming of residents, e.g. brushing hair, teeth, trimming fingernails and toenails. Monitor hygiene continuously and assist as needed to assure that each resident is clean and neat.

    If resident is unable to reposition self, aide will be required to turn and reposition resident intermittently.

    May be required to clean equipment such as bedpans, basins and empty catheter bags.

    Maintain each bedside unit in neat and orderly condition. Clean table surfaces as needed. Store personal items such as dentures, eyeglasses, combs, brushes in sanitary and convenient manner in proper containers.

    Make continuous observation of housekeeping and maintenance needs, e.g. broken fixtures, burned out light bulbs, soiled floors and curtains, and report needs promptly.

    Maintain bedside care supplies, e.g. gloves, tissues, toothpaste, use charge system to record supplies obtained from facility stock. Notify Charge Nurse to have them call if items are to be supplied by family.

    Maintain all service areas in clean and orderly condition.

    Assist in assuring the maximum quality of life for all residents maintaining privacy, accommodating individual preference, promoting independence, encouraging and assisting participation in facility activities and events and communication respectfully at all times.

    Respond to all call lights promptly. Keep call light within reach of all residents when unattended.

    Maintain continuous communication with team leader. Participate in report; communicate all changes and unusual observation concerning residents physical and psychosocial functioning.

    Attend unit meetings as scheduled, all mandatory in-services and optional in-services per individual needs and interest, for a total of at least 12 hours annually. Outside educational offerings may be substituted for optional in-service hours by providing proof of attendance. Mandatory in-service cannot be substituted.
