    Position: Case Manager

    Location: On-Site

    Requirements:

    Minimum one year of case management experience working with individuals experiencing homelessness. Bilingual proficiency in English and Spanish is required.

    Responsibilities:

    1. Direct Reporting and Collaboration:

    Report directly to the Program Director.

    Attend staff meetings, one-on-one meetings with the Program Director and Director of Social Services, case conferences, trainings, and community events as required.

    1. Member Support and Service Coordination:

    Provide medical oversight, including coordinating member appointments for general health, mental health, substance use disorders, etc. Schedule and coordinate home health services promptly.

    Order and arrange delivery of durable medical equipment (DME).

    Assist with applications for identification cards and income-related benefits such as SSI/SSDI, CAPI, General Relief/Calfresh, and job resources.

    Facilitate member admission and discharge processes, including completing admission notes and discharge summaries.

    1. Documentation and Compliance:

    Clearly explain program agreements and rules upon entry.

    Maintain accurate and complete member files, including daily progress and services documentation in the Electronic Medical Record (EMR) system.

    Hold one-on-one meetings with members to provide progress updates and address issues related to their residency and compliance with program rules.

    1. Crisis Management and Advocacy: Manage crises and intervene as necessary. Advocate for members to address barriers to medical care and resource acquisition.

    Establish and maintain collaborative relationships with mental health agencies, government entities, and other social services for referrals.

    1. Logistics and Transportation:

    Assist members with transportation needs to medical, mental health, and other health-related appointments.

    Ensure members are "document ready" for housing applications, assisting with gathering necessary documents like ID, SSN card, birth certificate, etc.

    Manage travel logistics for site visits and complete mileage reimbursement forms.

    1. Additional Duties:

    Perform other tasks, projects, and responsibilities as assigned by the Program Director.

    Work Environment:

    This is an onsite role requiring travel to various sites.
