    Job Summary: We are seeking an assistant to our current Care Coordinator to join our team. In this role, you will work collaboratively with a Care coordinator assisting patients in determining their medical needs, developing the best course of action, and overseeing their treatment plans, ensuring each client gets high-quality, individualized care. You will work one-on-one with clients as needed and provide care following their care plan. In addition, you will be responsible for providing transportation using the company vehicle. The ideal candidate is compassionate, reliable, trusted, and knowledgeable about healthcare practices.

    Caregiving: Individuals will be required to be on call during working hours and cover shifts as needed. Shift responsibilities are as follows but not limited to:

    Housekeeping

    Laundry

    Shopping

    Cooking

    Personal Care

    Companionship

    Outings and Appointments

    Weekend and Evenings availability

    On-call to provide transportation using company vehicle as needed for clients

    Care Coordinator :

    Collaborate with physicians, patients, families, and healthcare staff

    Review Care Plan to ensure it is being followed if not, update/ train accordingly

    Develop individualized care plans

    Accompany clients to Doctor / Hospital visits

    Create goals and monitor progress toward goals

    Recruit and train staff

    Create schedules

    Daily review of caregiver notes reporting changes in condition, issues, and concerns in the daily

    Qualifications

    Previous experience as a Care Coordinator or in a similar position is preferred

    CNA preferred

    Strong problem-solving and organizational skills

    Ability to manage multiple projects or tasks and prioritize appropriately

    Ability to work in fast-paced situations and make sound decisions quickly

    Excellent interpersonal skills and high level of compassion

    Strong verbal and written communication skills

    Basic knowledge of Windows computers

    Bilingual in Spanish

    Ability to lift up to 50lbs
