  • Responsibilities

    GLIN IPA is looking for a dedicated Care Coordinator to support our highest-risk patients as part of our interdisciplinary Care Management team. Under the clinical direction of Nurse and Social Work Care Managers, the Care Coordinator will be key to improving patient health outcomes by assisting with care coordination efforts. To address patient needs and expedite appropriate, cost-effective care, the Care Coordinator will assist with telephonic outreaches to patients to engage and enroll in GLIN IPA Care Management programs and support the Care Management team with needed provider and community agency collaboration. The Care Coordinator will also be responsible for administrative/clerical tasks such as preparing Electronic Medical Records (EMRs), requesting/uploading/downloading patient health information, faxing and mailing letters, and answering inbound calls to the Care Management queue.

    Duties and Responsibilities (including but not limited to)

    • Address patient obstacles to care
    • Facilitate engagement between primary care provider and patient
    • Daily collaboration with Care Management Team
    • Work as patient advocate
    • Performs additional duties as assigned
    • Answer inbound calls to the Care Management queue
    • Place outbound and/or engagement calls as needed
    • Prepare patient charts for potential outreach
    • Establish patient eligibility for Care Management Programming
    • Utilize multiple systems to gather information
    • Document all communications appropriately in EMR

    Qualifications or Education, Training, and Experience

    • High school or GED diploma
    • Two (2)+ years of work experience in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, Care Management, Home Health Care or related
    • Must have the ability to travel to and from the office for required training/ meetings

    Knowledge and Skills/Expected Competencies

    • Multitasking and prioritization working in a high-volume environment
    • Understanding and implementation of HIPAA and Confidentiality requirements
    • Excellent interpersonal and organizational skills
    • Compassionate and patient-centric approach
    • Adaptable to new technologies and software
    • Proficiency (including documenting in) in EMR system(s), Outlook, basic Excel

    *This is a hybrid-remote position, and candidates must reside in Western New York to be considered.

    Pay range $18.00 to $24.00

    The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range and determined by considering factors including, but not limited to, market data, education, experience, qualifications, and expertise of the individual, as well as internal equity considerations.
