    Care Coordinator

    Our client is in beautiful Fort Myers, FL and looking for qualified professionals to join their team! They are a well establish non-profit that brings much needed services to the community.

    The Position: Our client is looking for a Care Coordinator to join their established team in the Fort Myers area. They are offering the following compensation and benefits:

    • $16.50/hr-$23.47/hr Based On Experience
    • $1,000 Sign On Bonus
    • Hourly Intensive Differential

    Requirements:

    • Minimum Bachelor’s Degree in Social Work, Psychology, Rehabilitation, Special Education, Health Education, or a related field
    • 1 Year Experience Working with Substance Abuse or Co-Occurring Disorders
    • Proficient in Windows Operating Systems- Required
    • Ability to Learn Electronic Health Records (EHR)
    • Excellent Computer Skills

    Responsibilities:

    • Successfully works with patients with substance abuse or co-occurring disorders in an effort to effectively link them to needed resources, coordinate such resources as necessary, and monitor the implementation of such. Examples of resources may include, but are not limited to, recovery-oriented support systems, psychiatric services, substance abuse therapy and treatment services, housing, financial, vocational, connection to social activities, and other resources as needed to promote independence and enhance daily living.
    • Routinely advocates on behalf of the patient in order to obtain and maintain needed resources; advocates in a manner that promotes patient independence by encouraging and teaching patients to advocate for themselves.
    • Works closely with Detox, local hospitals, physician groups, internal programs, and other community agencies to collaborate, facilitate and advocate for services as needed for patients.
    • Effectively ensures that staff collaboration and communication is maintained with treatment facilities and providers at minimum once per month, assigned staff are involved in the discharge planning process, as well as ensuring I-INHU Team completes frequent attempts to communicate with identified participants 3x per week for 14 days.
    • Efficiently conduct a minimum of monthly contact by face to face, ZOOM or telephone, when possible complete a home visit every other month in order to provide support and conduct ongoing assessment of needs and monitoring of service plan needs.
    • Consistently complies with mandates of: CARF, DCF, 651)-30, CFBHN, Substance Abuse and Mental Health and policies and procedures and routinely demonstrates a sound understanding of and commitment to compliance with these standards.
    • As required, will be included in discharge planning, treatment team meetings, conference calls, and other treatment or rehabilitation facilities as needed or mandated by programs.
