Responsibilities:
Conduct care consultations with prospective clients alongside RN staff, addressing their needs, answering questions, and building relationships with clients and families.
Coordinate and oversee care services, including caregiver selection, scheduling, care plan reviews, caregiver orientation, and supervision.
Provide ongoing care management, ensuring service quality, client and caregiver satisfaction, and handling scheduling changes and updates.
Monitor and update care plans based on client needs, communicate changes, conduct home visits, and collaborate with RN staff for reassessments.
Manage administrative duties, including documentation, policy communication, caregiver training coordination, and participation in company meetings.
Maintain office hours, provide after-hours/on-call support as needed, and represent the company professionally in the community.
Job Requirements:
Healthcare experience required. Experience in patient care coordination preferred; senior (geriatric) care preferred. LPN or therapist (PT/OT) license strongly considered.
Knowledge of medical conditions and terminology, particularly for older adults
Available for work some weekends and evenings
Proficient with using software, mobile technology, data entry, and correspondence.
Ability to interface effectively and professionally with client stakeholders including family, healthcare providers, attorneys, etc.
Ability to function in a small team environment as well as autonomously
Excellent written and verbal communication skills
Travel in OKC metro area required. Mileage reimbursement is provided for job-related travel.
Must be comfortable working in homes with pets
Possess a valid driver's license and have use of an insured automobile
Clean criminal background
Minimum of 3 professional references
