Care Coordinator for High-Fidelity Wraparound Services (Intensive Care Coordination)
Job Description
Position Summary:
The Care Coordinator is responsible for providing high-quality, strength-based, and family-centered support as part of a wraparound service team. They will work closely with clients, families, and multidisciplinary service providers to develop and manage individualized care plans that focus on achieving positive outcomes. The role involves identifying needs, facilitating access to services, monitoring progress, and advocating for the best interests of clients within a collaborative framework.

Qualifications
• Education: Bachelor’s degree in social work, Psychology, Counseling, or a related field (master’s degree preferred).

Pay: $35,000.00 - $40,000.00 per year
