    Job Description

    Position Summary:
    The Care Coordinator is responsible for providing high-quality, strength-based, and family-centered support as part of a wraparound service team. They will work closely with clients, families, and multidisciplinary service providers to develop and manage individualized care plans that focus on achieving positive outcomes. The role involves identifying needs, facilitating access to services, monitoring progress, and advocating for the best interests of clients within a collaborative framework.

    Key Responsibilities:

    1.Client Engagement and Assessment

    • Engage with clients and families to establish trust and gather comprehensive information.
    • Conduct needs assessments to identify strengths, resources, and barriers affecting the family.
    • Develop culturally responsive strategies to address the individual needs of each family.
    • Carry a case load of 10 families.

    2.Care Planning and Coordination

    • Work with clients and their families to develop personalized care plans that address mental health, education, physical health, and social needs.
    • Coordinate services across various providers (mental health, medical, educational, social services).
    • Collaborate with wraparound team members, including social workers, therapists, educators, and community agencies, to implement and adapt care plans.

    3.Advocacy and Support

    • Advocate for clients and families within different service systems to ensure they receive appropriate support and resources.
    • Provide ongoing support and guidance, empowering clients to achieve self-sufficiency.
    • Facilitate family meetings and care team meetings to review progress and adjust goals as needed.

    4.Monitoring and Evaluation

    • Track and document client progress, adjusting care plans and interventions based on evolving needs and goals.
    • Maintain accurate and timely case documentation, including care plans, progress notes, and service updates.
    • Utilize data and outcome metrics to evaluate program effectiveness and report to supervisors.

    5.Community Collaboration and Relationship Building

    • Develop and maintain strong partnerships with community resources, service providers, and support networks.
    • Stay informed about local resources, services, and policies that impact clients and advocate for improvements as needed.
    • Participate in training and professional development to stay current with best practices in wraparound and coordinated care.
  • Qualifications

    Qualifications

    Education: Bachelor’s degree in social work, Psychology, Counseling, or a related field (master’s degree preferred).

    Experience: 2+ years of experience in case management, care coordination, social services, or mental health settings; experience with wraparound services is a plus.

    Skills Required:

    • Strong communication, organizational, and time-management skills.
    • Ability to work collaboratively and effectively with diverse populations.
    • Knowledge of community resources, family support systems, and behavioral health services.
    • Crisis intervention and conflict resolution skills.
    • Proficiency in documentation and case management systems.

    Additional Information

    Working Conditions: This position may require travel within the community, flexible hours, and participation in on-call rotation, depending on client needs.

    Job Type: Full-time

    Pay: $35,000.00 - $40,000.00 per year
