About the Role:

We are looking for a detail-oriented Care Coordinator to manage service schedules, case coordination, and staff assignments. This position ensures that youth receive the best possible care and support.

Key Responsibilities:

✔️ Assign and manage 1:1 staff schedules. ✔️ Work closely with caseworkers, caregivers, and behavioral staff. ✔️ Keep accurate service records and reports. ✔️ Provide problem-solving and service coordination.

Requirements:

🔹 Bachelor’s degree in Human Services, Social Work, or related field. 🔹 2+ years of experience in case management or youth services. 🔹 Strong communication & organizational skills. 🔹 Familiarity with child welfare services and support programs.
