    Are you passionate about providing quality care and ensuring a positive experience for both clients and caregivers? If so, we’re looking for a dedicated Care Manager to oversee the quality and consistency of our home care services in Duval and St. Johns Counties. I

    This is a part-time position, approximately 30 hours per week, depending on the caseload. We will collaborate with you to determine a schedule that works best for both you and the needs of our clients. In this role, you’ll visit our clients in their homes to assess the quality of care being provided and ensure that care plans are designed to meet their needs. You will also provide professional support and guidance to caregivers, ensuring that client care plans are executed to our high standards. You’ll work closely with caregivers, advising them on best practices, and regularly liaise with home health, hospice, and therapy professionals to coordinate comprehensive, client-focused care.

    Your day-to-day will involve conducting assessments, managing care plans, and maintaining open communication with clients, their families, and the broader care team. You'll also review documentation to ensure consistency and accuracy across all client records. As a key player in our team, you’ll help recruit, train, and coach caregivers to deliver top-tier care to our clients.

    Key Responsibilities:

    Client Care Oversight: Conduct in-person client visits, service check-ins, and phone assessments to ensure client needs are met and care plans are followed.

    Client Onboarding: Perform new client assessments, develop care plans, and ensure proper documentation for a smooth onboarding process.

    Support for Caregivers: Provide 1:1 coaching, advise on best practices, and monitor care to maintain quality and consistency.

    Coordination with Care Team: Work with home health, hospice, and other healthcare professionals to ensure comprehensive care.

    Record Keeping & Compliance: Maintain accurate client records and ensure compliance with state regulations.

    Qualifications and Requirements:

    Licensed Practical Nurse (LPN) Required.

    Minimum 2 years of experience in home care, hospice, or a related healthcare setting.

    Strong interpersonal skills with excellent attention to detail and organization.

    Experience in patient records management and client charting.

    Ability to self-direct and solve problems independently.

    Must be a self-starter with excellent telephone communication skills, capable of developing and managing a schedule for client visits and performing service quality reviews both in person and over the phone.

    Must have access to your own dependable vehicle for traveling to client homes
