    Our company works in partnership with the state agency, NJ Division of Developmental Disabilities. We perform case management/support coordination services for adults with developmental disabilities.

    The Successful candidates must be able to multitask and quickly transition between job responsibilities and be flexible to unanticipated changes. The job requires travel within Bergen, Hudson, Passaic Middlesex, Monmouth Morris, and Union Counties and candidates must have a dependable means of transportation. Salary is commensurate with experience.

    Responsibilities and Duties:

    · Work closely with the consumer and their family to learn about their skills, talents, and interests, and then link that individual to appropriate support and services in the surrounding community.

    · Draft and revise Individualized Service Plans that focus on the individual’s goals and aspirations

    · Serve as a liaison for consumers and their families and various statewide entities.

    · Monitoring the implementation of the plan with monthly phone contact, quarterly face-to-face visits, annual home visits, and subsequent reports/meetings.

    · Responding within 24-48 hours or less to all requests for information from individuals, service providers, family, state officials, and management regarding the members on your caseload.

    · Coordinate and facilitate team meetings off-site at either Residential and/or Day Programs to identify service needs.

    · Find service providers in the community to meet an individual’s needs.

    · Check to make sure the services are meeting an individual’s needs monthly.

    · A willingness to work a flexible schedule, as needed, to meet the needs of the people served.

    · On call to respond to emergencies.

    Qualifications and Skills:

    · Bachelor’s Degree a must.

    · Must have 1-2 years’ experience, either personal or professional, working directly with individuals with intellectual/developmental disabilities (experience can include paid employment, volunteer experience, and/or being a family caregiver of an adult with a developmental disability).

    · Proficiency with Microsoft Office applications and ability to learn other systems.

    · Valid driver’s license and ability to travel within Bergen, Hudson Passaic Middlesex, Monmouth Morris, and Union counties.

    · Commitment to the principles of Person-Centered Planning.

    · Must successfully complete State and Federal Criminal Background checks, Child Abuse Registry Information (CARI) checks, and Central Registry Check at the time of hire and successfully complete training required.

    · Ability to work with minimal supervision to meet or exceed deadlines.

    · Possess solid and effective spoken and written communication skills.

    · Ability to organize work effectively, establish priorities, and express oneself with a professional demeanor and presentation skills.

    · Ability to maintain compliance with policy and procedures and adherence to established deadlines and benchmarks.

    Work Setting:

    This role will take place in various settings. Work can occur at an individual’s home, day program, or place in the community. The coordinator is required to be flexible with their work schedule, including evening hours. The individuals and families we work with have a variety of needs. Salary Range: 46k-50k (based on the caseload)
