Job Description:

We are seeking a dedicated and compassionate Career & Family Services (CFS) Case Manager to join our team. As a CFS Case Manager, you will take on a dual role of being responsible for the client's service coordination as well as providing direct support to the client in activities of daily living (ADLs).

Key Responsibilities:

Provide case management services, including coordinating and monitoring services and supports.

Provide direct support to the individual in all areas of daily life, including food preparation, bathing, restroom use, medication administration, transportation, and recreation.

Conduct assessments and ongoing evaluations of the client's needs.

Collaborate with other service providers, community organizations, and agencies.

Develop and implement individualized service plans (ISPs) in collaboration with the client and their circle of support.

Advocate for client's needs to ensure they have access to necessary resources.

Maintain accurate, up-to-date case records and other documentation.

Participate in training and professional development activities to stay current with best practices and regulatory requirements.

Qualifications:

Associate's degree or higher.

Must be able to pass both a criminal DOJ/FBI LiveScan background check and DMV check.

Valid California driver's license and insured vehicle.

1+ year experience working with people with developmental and intellectual disabilities.

Valid CPR/First Aid certification.

Previous experience in case management or a related field preferred.

Working knowledge of state and local regulations related to case management and family services, or willingness to learn.

Strong written and verbal communication, organization, and problem-solving skills.
