    Client location and schedule hours:

    Client located in the Nashville are; Schedule is Monday, Wednesday and Friday

    Job Responsibilities: Social History: Retired Army Veteran from the Korean War. There is a Cat in the house . she keeps in a room with door closed in their house.

    Diagnoses: Diabetes Type 2 Insulin Dependent, Bilateral Cataracts, Glaucoma, Mobility weakness, Hyperlipidemia, High Blood Pressure, Gout, Rheumatoid Arthritis in knees. Mild Hearing Loss (no aids yet), Incontinence issues, some cognitive issues not specified Allergies: None to Food or Meds

    Medications: Metoprolol Succ ER 25mg 1x/day, Insulin Glargine (Inject 50 units @bedtime), Glipizide 10mg tabs 2x/day, Lisinopril 40mg 1x/day, Chlorthalidone 50mg 1x/day, Metformin HCL 1000mg 1x/day, Amlodipine 10mg 1x/day, Potassium Chloride 20 MEQ SA 1x/day, Allopurinol 100mg 2 tabs 1x/day, Finasteride 5mg 1x/day, Atorvastatin 80mg 1x/day, Vit D 50mg 1x/day, Latanoprost .0055 (1 drop ea eye daily)

    Goals & Objectives: Upon arrival, standby assist to the bathroom, observing if sheets need to be changed. Prepare & serve breakfast and assist him to take his morning meds. Remind/assist him to brush his teeth after breakfast and after lunch every shift. Do laundry as needed, change bed linens as needed. Encourage and offer water to stay hydrated.

    ADL's: Mobility: Client has a little weakness walking, so he is supposed to use a walker on all transfers. He has good upper arm strength and can rise to stand on his own. He walks at a normal pace without shuffling noticed.

    Toileting: Client wears depends. He knows when he needs to use the bathroom but leaks and sometimes does not get there in time. Prob good for CG to ask him if he needs to go a few times during shift.

    Bathing/Dressing/Grooming: Client showers independently using his support bars and shower chair. He does not like to get showers & has been giving himself "bird baths" at the sink. He's fine with CG doing standby assist. He can help choose his clothes for the day. He can brush his own teeth but needs encouragement and reminders.

    Eating: Client is a good eater and has no known allergies to any foods. He uses utensils independently if food is prepared and served to him. He does not like to drink water, so he should be given and encouraged to stay hydrated to prevent further UTI's.

    Communication: Client has good receptive and expressive verbal skills. He is a very quiet man and only speaks when spoken to. He is clear and articulate in his speech. He does have a hearing loss but does not yet have aids.

    Qualifications: Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member Ability to effectively communicate through written and verbal language Valid Driver's License and adequate auto insurance Consent to criminal, elder abuse, motor vehicle and sex offender background checks Consent to drug and alcohol testing TB test
