Client is male, in his 90's living alone in independent living. He is presently in a skilled nursing facility with expected discharge of Thursday, June 27th. Care will consist of assistance with personal care, safety, assistance with ADL's like dressing, some light assistance around apartment (there is housekeeping but he needs help with things like laundry) and companionship.
