Job Description: You will be responsible for coordinating and managing the care of patients in various healthcare settings. This is an important role in ensuring the well-being and quality of care for patients. Providing high-quality care to patients and making a positive impact on their lives
Major Job Duties: Maintain Client Confidentiality, Developing Healthcare Plans, Developing Client Goals, Client Management, Monitoring and Evaluating Client Progress, Ensuring Continued Compliance, Empathizing with Client Situations, Manage and Coordinate Client Care, Health Promotion, Administer Comprehensive Assessments, Outreach for ECM Program, Facilitating Community Resources, Improving Care Coordination, Individual and Family Support, Coordinate Transportation to Doctor's Appointments, Other Duties as Assigned.
Job Responsibilities: Conduct patient assessments to determine their healthcare needs and develop individualized care plans. Collaborate with healthcare providers to coordinate and manage patient care. Monitor patient progress and make necessary adjustments to care plans as needed.
Work with insurance company to communicate as needed regarding patient care.
Advocate for patients' rights and ensure they receive appropriate care. Maintain accurate and up-to-date documentation of patient interactions and care plans.
Qualifications: Education: HS Diploma or GED Equivalent. Experience: Customer Service background required. Knowledge of HIPAA a plus. 6 months minimum of Case Management or Medical assisting preferred. Additional Qualifications: Some Travel Requirements, Strong Comprehension of Cultural Competency, Computer Literacy. Must have valid driver's license and clean driving record
