    Simple Solutions is hiring a Case Management Coordinator. They will be responsible for coordinating and managing the care of patients, ensuring their well-being and making a positive impact on their lives. The case management coordinator will conduct patient assessments to determine their healthcare needs and develop individualized care plans, making necessary adjustments as needed. They will be advocating for patients’ rights and ensure they receive appropriate care while maintaining accurate and up-to-date documentation of patient interactions and care plans.

    Essential Functions

    Respect Client Confidentiality: All case notes, member records and personal information must be kept confidential.

    Advocacy: Advocate for the best interests of their members, ensuring they have access to necessary services and resources.

    Clear Communication: Always ensure that members are informed about their options, processes, and what they can expect. Regular follow-ups with members are standard.

    Timely Updates: CCA/Care director should be updated regularly and on time, particularly after speaking with the members or any significant progress or changes.

    Accurate Documentation: Maintain detailed, accurate and objective documentation. Avoid vague terms and focus on facts.

    Compliance with Legal and Ethical Standards: Follow all relevant regulations related to record-keeping, such as HIPAA or other local laws governing member confidentiality.

    Caseload Management: Manage a caseload of 75 members.

    Efficiency: As a Case Management Coordinator, you are expected to complete HRAs and care plans efficiently and within required time frames.

    Requirements:

    High School Diploma

    1-2 years case management

    Basic computer skills

    Good communication skills such

    Ability and willingness to provide emotional support, encouragement, and motivation to members.

    Licensed driver with automobile insurance in accordance with the state

    Good driving record

    Knowledge and use of medical terminology preferred
