POSITION SUMMARY
The Case Manager, will develop and nurture therapeutic relationships with students and their families; assure all mandated counseling services as described by a student’s Individualized Education Plan are received; deliver program driven related services; maintain appropriate communication with parent/legal guardians; evaluate and report information appropriate to individual plan development and maintenance, where appropriate, participate in linkages between the residence and outside social services, where appropriate, coordinate, secure, evaluate, and report applicable social service information; develops, supports, and maintains continuity of services between the residence and outside social services.
The Case Manager’s Duties and responsibilities include, however are not limited to the following:
· Intake-ensuring all paperwork for admission of a new client is complete and responsible for ensuring that all pertinent admission information is obtained-Social, Birth Certificate, photo, etc.
· Ongoing communication and collaboration with all members of the County Case Worker, probation officer and family as appropriate
· Record Keeping-Developing and setting up the Chart in a consistent and uniform manner
· Properly thinning out records which are full and storing the paperwork according to ADVANTAGE CARE GROUP’s Record Management Policy
· Record Keeping-ensuring all notes and SBR’s are accurate and reflective of the identified goals, all paperwork is consistent and filing documentation in the record accurately and timely manner
· Scheduling and documenting appointments as they occur
· Preparation of Reauthorization packages/ and Submission
· Providing assistance with discharge planning-maintaining correspondence with discharge resources/ providing documentation as requested
· Accompanying Clients to court, medical appointments and School meetings
· Documenting all Client interactions in which the Case Manager is directly involved
· Responding to phone calls/ Some on call duties and responsibilities
· Prepare incident Reports for Director’s Signature
· Responsible for the packaging and storage of closed files
· Entering data on the outcomes Report
· Maintaining general office supplies
WORKING CONDITIONS
Working environment is generally favorable; there is the potential for adverse and/or hazardous events related to working with the youth. Advantage Care Group is located in a private community setting.
PHYSICAL DEMANDS
Position often requires lifting up to 10 lbs. and intermittent sitting, standing, bending and walking; requires good visual and auditory acuity for monitoring youth behavior. In an emergency, employee must be able to physically respond and assist in the evacuation or rescue, bend, lift, run or walk swiftly in order to intervene in a combative situation(s) to protect fellow employees or other youth from the threat of harm or actual harm at the hands of youth. Must have the ability to administer First Aide and CPR.
SPECIAL TRAINING/SKILLS
Thorough understanding of age, cognition, and developmental stages of school age children. Effective communication skills and ability to quickly form trusting relationships with child/adolescent population. Attend and participate in all in-service training programs.
