    Job Overview:

    The Case Manager Assistant in a home health setting provides administrative and logistical support to case managers and healthcare teams. The assistant helps coordinate patient care, manage documentation, and ensure that patients receive the appropriate services and resources while receiving care at home. This role is vital in streamlining patient care delivery, maintaining communication between the healthcare team and patients, and providing essential administrative support to case managers.

    Key Responsibilities:

    Support in Case Management:

    Assist case managers in coordinating patient care by scheduling appointments, follow-up visits, and home health services.

    Help maintain patient files and ensure all necessary documentation is accurate and up to date.

    Track and follow up on patient care plans, ensuring that services are delivered as prescribed and that patients are compliant with their care regimens.

    Patient and Family Communication:

    Serve as a point of contact for patients and their families, providing updates on appointments, services, and care plans.

    Ensure that patients and family members understand their treatment plans, scheduled appointments, and the services provided by home health agencies.

    Answer phone calls, emails, and other communications from patients, family members, and healthcare providers.

    Documentation & Record Keeping:

    Assist with maintaining accurate patient records in compliance with healthcare regulations and company policies.

    Help case managers with the preparation of patient documentation, including care plans, assessment forms, and progress notes.

    Ensure timely and accurate entry of patient data into electronic health records (EHR) systems.

    Coordination of Services:

    Assist in coordinating home health services, such as nursing visits, physical therapy, and other medical services.

    Communicate with healthcare providers, insurance companies, and other stakeholders to ensure the smooth delivery of care services.

    Monitor the delivery of home health services to ensure that the patient’s needs are met and document any issues or concerns.

    Scheduling and Coordination:

    Schedule home health visits and services in coordination with the healthcare team and patient preferences.

    Ensure that all appointments and follow-up visits are confirmed and that patients have the necessary information for the visit.

    Assist with scheduling patient transport when necessary.

    Resource Referral and Advocacy:

    Help patients and families access community resources, including home health aides, transportation, or financial assistance programs.

    Assist with arranging referrals to specialists or other healthcare professionals as needed.

    Provide information and assistance on insurance coverage, eligibility, and benefits related to home healthcare.

    Assisting with Compliance and Quality Assurance:

    Help monitor patient care and assist with ensuring that healthcare standards and regulations are met.

    Work with case managers to address any gaps in service or areas that need improvement to enhance patient care and compliance.

    Other Administrative Tasks:

    Prepare reports and assist in tracking the progress of individual patients and overall case load management.

    Support case managers with general administrative duties such as filing, copying, and organizing patient care documents.

    Attend team meetings and collaborate with other members of the healthcare team to improve patient care.

    Required Qualifications:

    Education: High school diploma or equivalent required. Some college coursework or a degree in healthcare administration, nursing, or a related field is preferred.

    Experience: Prior experience in home health, case management, or healthcare administration is preferred but not required.

    Skills:

    Strong organizational and time-management skills.

    Good communication skills, both verbal and written.

    Ability to maintain patient confidentiality and follow HIPAA regulations.

    Familiarity with medical terminology, healthcare processes, and insurance.

    Proficiency in using computers, electronic health records (EHR), and office software (Word, Excel).

    Ability to work well independently and as part of a team.

    Preferred Qualifications:

    Experience in home health or healthcare-related settings.

    Basic knowledge of insurance, billing, and coding.

    Certification in medical office management or case management is a plus.

    Work Environment:

    The Case Manager Assistant will primarily work in an office setting but may occasionally visit patients' homes as needed to ensure coordination of care.

    Occasional travel may be required for home visits or meetings with healthcare providers.

    This is typically a full-time position, with regular office hours. Some flexibility in hours may be required depending on patient needs.
