    Job Summary We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families in navigating complex systems of care. This position involves assessing client needs, developing tailored service plans, and providing ongoing support to ensure positive outcomes. The Case Manager will work collaboratively with clients, families, and community resources to facilitate access to services and promote overall well-being.

    Responsibilities

    Conduct comprehensive assessments to identify client needs and strengths.

    Develop, implement, and monitor individualized service plans.

    Provide crisis intervention and management as needed.

    Utilize motivational interviewing techniques to empower clients toward positive change.

    Collaborate with community organizations and resources for workforce development and support services.

    Offer addiction counseling and support for clients dealing with substance use issues.

    Supervise casework activities, ensuring compliance with agency policies and best practices.

    Maintain accurate documentation of client interactions, progress notes, and service delivery.

    Advocate for clients in various settings, including child welfare systems and healthcare providers.

    Mentor clients in developing life skills and achieving personal goals.

    Experience Candidates should possess a background in social work or a related field, with experience in the following areas considered beneficial:

    Crisis management techniques

    Supervising staff or volunteers

    Mentoring individuals or groups

    Bachelors or Masters Degree Required
