    Case Manager Job Responsibilities:

    1. Admits new clients by reviewing records and applications; conducting orientations

    2. Prepares clients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements.

    3. Writing ISP programs and safety plans for clients

    4. Reviewing monthly ISP data for each client

    5. Treatment integrity evaluations over SLP and DSP staff

    6. Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends

    7. Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to the therapeutic team and family; identifying treatment influences.

    8. Completing Comp assessments for clients.

    9. Conducts Meet and greets

    10. Attends all ISP meetings

    11. Keeps clients file up to date and in compliance with regulations

    12. Maintains clients’ records by reviewing case notes; logging events and progress.

    Case Manager Requirements and Qualifications

    · Possesses superb written and spoken communication skills

    · Excellent interpersonal skills with colleagues, community leaders, policymakers, and others

    · Excellent time management skills; organized and able to prioritize.

    · Motivated to take on additional community involvement projects and solve problems.

    · Comfortable in a fast-paced environment with multiple cases

    · Able to organize and manage large amounts of files, schedules, dates, and information.

    · Self-directed and able to work without supervision.

    · Comfortable with building personal relationships and dedicated to helping others

    · Empathetic and supportive with mentorship and leadership skills

    · Bachelor's degree in social work, psychology, sociology, healthcare or medical and related fields; master’s degree in social work preferred; supervised practicum or internship preferred

    · Licensed or certified by state board according to regulations.

    · Proficient computer skills, including Microsoft Office Suite (Word, PowerPoint, and Excel)

    · Able to work nights and weekends

    · Able to travel to meet with clients

    Education, Experience, and Licensing Requirements:

    High School Diploma or GED

    3+ years customer service experience, preferably in human services, possess demonstrated commitment to customer service.

    Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred.

    Must have a valid in-state driver’s license, provide current registration and insurance, and have a driving record with no more than 2 points within the last 3 years

    Must be over 21 years old
