    Job Purpose/Summary

    The primary responsibility of a Case Manager is to provide comprehensive support and assistance to individuals receiving services at the Sacred Path Childcare and Family Resource Center. Case Managers play a crucial role in helping clients access the resources and services they need to improve their well-being and overall quality of life. Case Managers collaborate with other members of multidisciplinary treatment teams to develop plans that address the client’s specific needs. (e.g., mental health, substance use, social, financial, academic, occupational, medical, cultural). The role involves providing linkages, resources, transportation, advocacy, and other forms of support.

    Essential Duties & Responsibilities

    Collaborate with a multidisciplinary team, including social workers, therapists, medical professionals, and community organizations, to provide comprehensive care for clients.

    Contact new clients within 24hrs and schedule the initial appointment within 5 business days.

    Develop personalized care plans tailored to each client's unique needs. Plans may include referrals to therapy, support groups, housing assistance, and other relevant services.

    Connect clients with appropriate community resources and services, ensuring they have access to the necessary support networks.

    Serve as an advocate for clients within the agency and in the broader community (e.g., school, Department of Child and Family Services, Probation, Children’s Court).

    Provide in-home and field services, as necessary and appropriate, to meet the needs of clients who may be experiencing a range of psychological emotional and behavioral problems. Services may include parenting, budgeting, social skills, and other types of support.

    Coordinate and facilitate cultural events and community/youth activities under Sacred Path Programs.

    Regularly assess clients' progress and adjust care plans as needed. Collaborate with the client and other professionals involved in their care to ensure a coordinated approach to services.

    Maintain accurate and up-to-date records of client interactions and interventions.

    Participate in community outreach efforts to raise awareness about culture, mental health issues and resources available at Sacred Path.

    Supervise children and youth participating in Sacred Path programs and events.

    Provide client transportation to Sacred Path events, appointments and linkages related to client’s overall plan of care.

    Administrative Responsibilities

    Submit required documentation within the timeframe required by the funding source (e.g., within 24hrs of client interaction).

    Ensure work schedule is updated and accessible to appropriate supervisor(s).

    Attend all mandatory meetings (e.g., staff, team, supervision) and training.

    Review and ensure client chart information is current.

    Communicate regularly with treatment team members regarding client services.

    Assist with Quality Assurance tasks, as needed, and respond in a timely manner when input or follow up is requested.

    Contact a supervisor in the event of a client crisis.

    Discuss plans for time off with supervisor to coordinate client and task coverage.

    Other duties as assigned or required to fulfill the mission of Sacred Path.

    Experience & Qualifications

    Minimum of 1 year experience in the provision of case management services in clinical or other human service settings.

    Familiarity with entities such as the Department of Child and Family Services, Probation Department and other child/family-related laws, programs, etc.

    Education

    High School diploma or Associate of Arts degree from an accredited institution.

    Patient Care and Customer Service

    Deliver exceptional customer service to clients and their families, addressing inquiries, concerns, and complaints in a timely and compassionate manner.

    Resolve any issues related to patient care or service delivery and escalate matters as necessary.

    Communication/Interpersonal Skills

    Interact with people in a professional and respectful manner, both verbally and in writing.

    Some key skills in this area include active listening, clear verbal/written communication, empathy, compassion, cultural humility, conflict resolution, problem-solving, collaboration, time-management, advocacy, resilience, adaptability and adhering to ethical standards such as confidentiality.

    Technology Skills

    Must be able to use a computer or laptop to complete needed tasks for a job.

    Excel, Word, Google Workspace (i.e., Sheets, Docs), iPad, electronic health record.

    Utilize technology tools for virtual meetings and remote monitoring to ensure continuity of care.

    Maintain accurate and up-to-date client records and progress notes within the EHR system; ensuring data accuracy, security, and adherence to privacy regulations to safeguard sensitive client information.

    Position Qualification Preferences

    Experience working with the American Indian/Alaska Native community.

    Physical Demands

    The physical demands described are typical of those required by an employee to perform the primary functions of this job successfully. While performing the duties of this job, the employee is required to stand, walk frequently, sit, bend, twist, talk, and hear. There are prolonged periods of sitting, keyboarding, reading, as well. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include reading, distance, computer, and color vision. Talking and hearing are essential to communicate with vendors, and staff.

    Mental Demands

    There are various deadlines associated with this position. The employee must also multi-task and interact with many people on a variety of issues that are at times, very complicated.

    Other Requirements

    Eligible to work in the United States

    Pass fingerprint clearance

    Maintain annual negative TB test results on file

    Pass a post-offer drug test

    Possess and maintain a reliable vehicle with valid CA driver’s license and active auto insurance

    Available to work evenings and weekends, as needed

    Willing to travel, as needed (travel time could range from 30 - 40%)

    Work remote temporarily due to COVID-19.
