Job Title: Case Manager
Status: Non-Exempt
Position Type: Part-Time 32 hours per week;
Must be able to work mornings, evenings and weekends.
SUMMARY:
Under the supervision of the Executive Director, the Case Manager provides case management through information and referral services to guests of Family Promise. This position also assists guests of Family Promise in organizing activities aimed at securing housing, employment and educational/vocational opportunities, physical healthcare, behavioral healthcare and childcare as needed.
EDUCATION AND EXPERIENCE:
Bachelor’s degree in human services or related field or equivalent work experience or combination of education and experience.
One (1) year of experience working with individuals who are homeless or similar populations.
Must possess a valid California Driver’s License and be able to provide proof of current auto insurance.
Must be able to present a current DMV printout of the prior three (3) year’s history.
Experience working in a non-profit setting preferred.
Ability to speak, read and write fluent Spanish preferred.
SKILLS:
· Ability to operate standard office equipment to including personal computer.
· Intermediate computer skills with working knowledge/experience to include Microsoft Office Suite.
· Knowledge of standard office practices and procedures.
· Excellent telephone /communication skills.
· Requires strong verbal, written, analytical, organizational and interpersonal skills.
· Ability to work independently and exercise confidentiality and sound judgment in making decisions.
· Ability to organize, prioritize and meet deadlines while working on numerous on-going projects simultaneously.
Provide creative and effective ideas for promoting the self-sufficiency of program guests.
Communicate and work collaboratively with program guests, staff, volunteers and partner agencies in a manner that reflects partnership, respect and mutual accountability in addressing difficult life situations.
Must have a positive attitude.
