    Job Summary Georgina Home Care is seeking a highly motivated and compassionate Care Manager to join our team in Waterbury, CT. As a Care Manager, you will be responsible for ensuring the well-being of our older adult clients in a home care setting. Your major end result will be to provide exceptional social services to our clients, including, mental health support, and long-term care management . You will work closely with our clients and their families to develop personalized care plans that meet their needs and preferences. Your role will be critical in ensuring our clients receive the highest quality of care possible.

    Responsibilities

    Collaborate with families , caregivers , case mangers and office staff

    Staff your case load with caregivers

    Develop individualized care plans

    Bi-monthly Home Visits to your clients

    Create goals and monitor progress toward goals

    Create schedules

    communicate with the state , client , families and caregivers.

    Qualifications

    Previous experience as a Care Coordinator or in a similar position is preferred

    Certification as a medical assistant is preferred

    Strong problem-solving and organizational skills

    Ability to manage multiple projects or tasks and prioritize appropriately

    Ability to work in fast-paced situations and make sound decisions quickly

    Excellent interpersonal skills and high level of compassion

    Strong verbal and written communication skills
