Job Description:
House of Hope, a nonprofit organization, is looking for a detailed-oriented and well-versed Case Manager to join our team! As a Case Manager your position is essential in coordinating the care and health management of the clients.
Experience, Knowledge, and Skills:
· Bachelor’s Degree Required.
· Two (2) years’ experience working in addictions/mental health.
· Ability to work with a diverse population.
· Effective communication skills
· Must be an independent thinker.
· Computer Literate
Duties and Responsibilities:
Collaborate with each client to develop their individual service plan.
Network with agency and outside service providers, ensuring client access to appropriate resources.
Orient clients upon phasing up, explaining all job search policies and procedures.
Complete required training and chart audits to ensure compliance and quality of services provided.
Communicate effectively with all treating professionals regarding clients.
Additional tasks and duties as assigned.
