  • Responsibilities

    Case Manager/Independent Living Specialist/Supportive Living Specialist/Parent Trainer. Provide assistance in the development and implementation of individual service plan (I.S.P.), including (but not limited to): independent living skills training; case management; intervention and advocacy; and write and submit quarterly and annual reports. Must have reliable transportation and full car insurance (proof of car insurance and car insurance policy) fore traveling to individual's home and transporting of individual. Occasional weekends and overtime may be required. Must be able to meet flexible schedule and be open to all assignments. Background check will be required.

    ESSENTIAL DUTIES:

    Conduct in-home training in daily living skills, incorporating the use of written, pictorial, tacticle, and video materials to augment instruction.

    Conduct community training in resources and transportation usage.

    Assist individual to access and use available entitlement benefits.

    Assist individual with completion of all reporting requirements for benefits.

    Monitor individual's health and medical needs.

    Identify home or community health and safety hazards and resources.

    Advocate for individual whenever necessary.

    Assist individual with development of personal goals and decision making.

    Provide problem solving support in social and interpersonal situations.

    Intervene in crisis situations, locate necessary resources, and coordinate services.

    Document all services and make recommendations for additional support.

    Complete current status reports and assist with development of service plans.

    Attend and contribute to individual's quarterly, semi-annual, and annual IPP meetings.

    Perform other related duties as required or assigned, especially with regard to serving and supporting the individual's needs.

    Skills Requirements

    Must possess the ability to communicate effectively, both verbally and in writing.

    Must have the ability to work with consumers' budgets and balance their accounts.

    Must be able to life up to 25 lbs.

    Experience working with developmental/intellectual disabled and/or dual diagnosed individuals is desirable.
