As a Case Manager, you will be responsible for providing comprehensive support and guidance to individuals and families in need. Your role will involve assessing client needs, developing and implementing individualized service plans, and connecting clients with appropriate resources and services. Your duties will include:
Complete intake and needs assessment with families, which may involve a broad range of areas including housing, finance & budgeting, employment, education, food & nutrition, school & daycare, legal issues, health, parenting, and daily living skills
Interview and evaluate families to formulate Individualized Service Plans based on Program requirements
Work closely with families to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process
Help families build self-sufficiency plans
Assist families to transition to self-advocacy roles
Advocate on behalf of families and attend administrative hearings, if necessary
Perform home visits and/or unit inspections
Establish, evaluate, and monitor goals for families linked to measurable outcomes
Work collaboratively with interdisciplinary team of collateral providers and partners to ensure coordination of services
Maintain the structure and policies of the program through orientation of families by reviewing program rules, policies, and case management visits
Maintain accurate case records by documenting all services rendered including up to date case notes, telephone contact logs and referrals
Enter required data into the agency databases with reporting, as required
Provide emergency response management to address any crises or safety issues that may arise
Accompany and/or transport participants to appointments as necessary
Assist families with scheduling appointments
