    πŸ₯ RN Case Manager – Pediatrics | $55/hr. Starting Rate Location: Washington, DC Schedule: 5x8 Days | Monday – Friday | 8:30 AM – 5:00 PM Company: Patient Centered Services, LLC

    Patient Centered Services is currently seeking a Registered Nurse Case Manager to support pediatric clients in the Washington, DC area. If you're passionate about pediatric care coordination, enjoy working with multidisciplinary teams, and are experienced in managing complex discharge planning, we’d love to hear from you.

    Position Requirements:

    Active DC RN license in good standing

    Bachelor of Science in Nursing (BSN) – Required

    Basic Life Support (BLS) – Required

    Minimum 5 years of case management experience

    At least 2 years of pediatric case management – Preferred

    Patient Diagnoses May Include (but are not limited to):

    Prematurity

    Bronchopulmonary Dysplasia (BPD)

    Cerebral Palsy (CP)

    Seizure Disorders

    Status Post (S/P)

    Motor Vehicle Accidents (MVA)

    Trauma / Post-Trauma

    Experience With:

    Discharge planning with community resources

    Collaboration with vendors and agencies

    Private Duty Nursing services

    Utilization review and cost-effective care planning

    Care coordination across multidisciplinary teams

    Required Skills:

    Strong understanding and application of Milliman Care Guidelines (MCG)

    Admission criteria evaluation

    Conducting concurrent reviews

    Determining and documenting medical necessity
