    Duties/Responsibilities:

    Works with a caseload of participants, including reentry population, to assist them in their journey towards becoming a productive, active member of the community.

    Works with individuals who are active in the streets as identified by the outreach workers and participates actively with Outreach team.

    Case Management includes, but is not limited to, support related to employment and education, life skills training, crisis intervention, relationship training, advocacy and support with involvement with justice system, and assessment and referrals to needed resources.

    Familiarity working with Care Coordination and Support Services (CCSO)

    Responsible for telephonic and/or face to face assessment, planning, implementing and coordinating care management activities with members to ensure that their medical and behavioral health needs are met and to enhance the member’s overall wellness.

    Conduct a comprehensive assessment using IM+CANS in addition to clinical tools and information to address the needs and approach to case resolution

    Using advanced clinical skills, performs crisis intervention with members experiencing a behavioral health crisis and refers them to the appropriate clinical providers for thorough assessment and treatment, as clinically indicated.

    Provides crisis follow up to members to help ensure they are receiving the appropriate treatment and services.

    Applies and interprets applicable criteria and clinical guidelines, standardized case management plans, policies, procedures, and regulatory standards while assessing benefits and/or member’s needs to ensure appropriate administration of benefits.

    Serves as a single point of contact for members and assists members to remediate immediate and acute gaps in care and access.

    Follows Treatment Plans created by Clinical Team

    Provides services in an individual, family, and group context

    Must be CPR certified

    Proficient in Microsoft Office Suite and comfortable using computer programs to track participant engagement.

    Ability to communicate in multiple settings and mediums (written, interpersonal, small group, and presentation) with great effectiveness.

    Ability to be on call 24 hours during your assigned rotation (Mobile Crisis Intervention Team)

    Maintain a high level of confidentiality

    *Bilingual is a plus

    Qualifications:

    Bachelor’s degree in social work, counseling, rehabilitation counseling, vocational counseling, psychology, pastoral counseling, family therapy, education or related human service field, or qualification of QMHP or MHP as declared by the State of Illinois

    Knowledge of the City of Chicago and communities with histories of violence

    Experience in social work, criminal justice, or a related field
