Case Manager Department: Shared Living Full Time Location: Springfield Schedule: Monday-Friday, 9a-5p Base Salary: $48,000–$50,000/year + $2,000 differential per additional case (See details below)

ServiceNet’s Shared Living Program is are seeking a dedicated and compassionate Case Manager to coordinate care for individuals living in Shared Living homes. This is a vital role within our team—ensuring each person supported receives the care, dignity, and individualized services they deserve.
Key Responsibilities:
Manage and oversee the support of individuals in Shared Living homes
Coordinate and monitor services provided by Care Providers, Individual Support Staff (ISS), Personal Support Staff (PSS), and Respite Providers
Ensure all services align with funding regulations, agency policies, and quality standards
Identify stressors and implement interventions to maintain placement stability
Supervise support staff and provide crisis intervention as needed
Collaborate with the Department of Developmental Services (DDS) and other agencies
Complete all required reports and assessments in a timely and professional manner
What You Bring:
Broad knowledge of human services or social work
Bachelor's degree in a related field preferred
3–5 years of progressively responsible experience
Strong organizational and communication skills
Computer proficiency (Microsoft Office and related systems)
Valid driver’s license and reliable transportation
Salary Details:
Starting Base: $48,000 -$50,000/year based on experience, education, and workload
Differential: $2,000 for each additional case
Minimum Caseload: 15
Maximum Caseload: 20
