    Job Description

    AHRC NYC is seeking to hire a Case Manager for their Manhattan Train & Earn Career Pathway Program with the NYC Department of Youth Community Development.

    ** Scope of Position:**

    Train & Earn is a career pathways program funded by the New York City Department of Youth and Community Development (DYCD) that provides comprehensive job training and employment services, along with support services needed by participants to find a permanent job, obtain their high school equivalency if applicable, and access postsecondary education and training. Train & Earn is for youth and young adults aged 16–24, who are not working and not in school, and meet certain eligibility requirements. The Case Manager oversees the overall coordination and direction of services for the people enrolled in the DYCD Train & Earn program.

    ** Essential Responsibilities:**

    The Case Manager will be responsible for the following duties within the DYCD Train & Earn program:

    • Coordinate support services for individuals on a caseload relating to employment success
    • Support participants in completing paperwork and administrative responsibilities for the program
    • Prepare service plans and other plans as appropriate or needed to support individuals in reaching their employment goals
    • Participate in planning for services by addressing social factors pertinent to development of employment plan, including financial planning and access to community resources
    • Conduct initial and on-going assessments, service plans, and progress notes with program participants
    • Conduct supportive services, either in-person or remotely, as needed and appropriate.
    • Participate as a member of the interdisciplinary team, attending case conferences, staff meetings, and parent meetings as appropriate.
    • Conduct personal and group meetings with the people supported and their network of supports.
    • Monitor progress of all participants on caseload, tracking program activities and outcomes
    • Promote program engagement, track training attendance and regularly assess training results of the people supported.
    • Conduct reviews of the expectations of the program and provide guidance to people supported, family and other service providers involved. Provide support with obtaining industry certification.
    • Deliver Work Readiness, Occupational Training and other program related curriculum when needed.
    • Create and maintain excellent relationships with EBS staff for purpose of preparing job seekers for success in employment.
    • Provide coverage within the program in event of excessive staff absences.
    • Perform other duties as assigned.
  • Qualifications

    Qualifications

    • Bachelor's degree in social work or a related field and experience working with City systems such as public assistance, child welfare, education, and housing; OR
    • An Associate’s degree and experience working with City systems such as public assistance, child welfare, education, and housing; OR
    • At least three (3) years successful experience in providing case management/counseling and working with City systems such as public assistance, child welfare, education, and housing.
    • Ability to communicate effectively, both verbally and in written form.
    • Ability to establish and maintain effective working relationships with participants’ families, employers, funding sources, Care Coordinators, and other stakeholders.
    • Ability to organize work of self and others, as well as work independently as needed.
    • Experience or strong capacity in instructing/mentoring program participants, with prior work exposure in teaching or training individuals highly desirable.
