    Qualifications:

    Minimum of 2 years of experience in case management or a related role.

    Bilingual in Spanish and English

    Strong understanding of social services, community resources, and case management principles.

    Excellent communication, interpersonal, and organizational skills.

    Ability to work effectively with diverse populations and handle sensitive situations with compassion and professionalism.

    Proficiency in Microsoft Office and excel.

    Valid driver’s license and reliable transportation.

    Lived experience navigating the “system”

    Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.

    Experience working with individuals at risk of homelessness, eviction, or financial instability.

    Position Summary: HavenSafe is dedicated to preventing homelessness in Santa Clara County by connecting at-risk individuals and families with vital resources, including eviction support and rental assistance. We are seeking a compassionate and dedicated Case Worker to join our team. The Case Worker will play a crucial role in supporting clients to remain housed, educating stakeholders, and fostering collaborations within the community to create a more efficient ecosystem of homelessness assistance.

    We are currently seeking a team member who is fluent in English and Spanish.

    Key Responsibilities:

    Client Assessment and Intake:

    Respond to inbound calls promptly, present an overview of the program to each prospective client in a way that builds trust, and conduct preliminary assessments for eligibility.

    Conduct in-depth assessments to gather information from each client as an input to an application for rental assistance.

    Develop individualized service plans in collaboration with clients to prevent eviction and homelessness.

    Case Management:

    Provide ongoing support and case management services to clients, including coordinating follow-ups regarding financial planning.

    Coordinate with other service providers and agencies to ensure comprehensive care.

    Advocacy and Support:

    Advocate for clients' rights and needs within the community and service systems, emphasizing eviction prevention and financial support.

    Provide emotional support and crisis intervention as needed.

    Empower clients by providing education on their eviction rights and available financial resources.

    Documentation and Reporting:

    Maintain accurate and up-to-date client records, including case notes, service plans, and progress reports.

    Prepare and submit regular reports on case management activities and client outcomes.

    Team Collaboration:

    Work collaboratively with other case workers, volunteers, service providers, and community partners.

    Participate in team meetings, training sessions, and professional development opportunities.

    Compliance and Ethics:

    Adhere to HavenSafe’s policies and procedures, including confidentiality and ethical standards.

    Ensure compliance with all relevant laws, regulations, and funding requirements.

    Work Environment:

    The position may require working in various environments, including office settings, client homes, and community locations.

    Occasional evening or weekend work may be required to meet client needs.
