    Job Description:

    People’s Path Inc. is a nonprofit organization: We are currently looking for an experience case worker that will be utilizing their educational knowledge while building experience on the job under the supervision of our senior staff members to provide comprehensive case management services to our clients in need. This includes assessing clients’ needs, developing individualized service plans, and coordinating with various resources and services to help our clients achieve their goals.

    The case worker will work closely with clients, their families

    Key Responsibilities:

    -Client Assessment: Conduct initial assessments to determine client’s needs, strengths and barriers. Develop individualized service plans in collaboration with clients.

    -Case Management: Provide ongoing case management, including follow-up, monitoring progress and adjusting service plans as needed.

    -Resource Coordination: Connect clients with community resources, including housing assistance,

    Mental health services, substances abuse treatment, assisting clients to find a job, and financial assistance.

    -Advocacy: Advocate on behalf of clients to ensure they receive appropriate services and benefits. Assist with applications for public assistance and other relevant programs.

    -Documentation: Maintain accurate and confidential client records, including case notes, service plans, and progress reports. Ensure all documentation complies with agency policies and confidentiality requirements.

    -Crisis Intervention: Provide support and intervention in crisis situations, including safety planning and immediate resource access.

    Other duties may be assigned.

    Qualifications:

    • Bachelor’s degree in social work, human services or psychology, or a related field.

    • Minimum of 2 years of experience or more in the field of social work

    • Excellent communication, organizational, and problem-solving skills.
