    Job Summary

    We are looking for a Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

    Responsibilities

     Perform personal care activities as outlined in a written assignment.  Enhance clients' lives by providing tailored care and support, ensuring comfort and safety.  Conduct light housekeeping, including laundry, dusting, dishwashing, and vacuuming.  Prepare and assist with feeding meals as needed.  Assist patients with ambulation and maintaining a safe, healthy environment.  Monitor and record temperature, pulse, and respiration.  Promote mental alertness through engaging activities and provide emotional and psychological support to patients, families, and caregivers.  Observe, report, and document patient status, including changes in mental or physical condition or home situation, to the VITAS Nurse and interdisciplinary team.  Teach basic personal care to primary caregivers and practice basic infection control measures.  Assist with treatments as ordered by the physician, supervised by a nurse, including:

    Changing colostomy bags.

    Assisting with walkers or wheelchairs.

    Reinforcing dressings.

    Performing prescribed range-of-motion exercises.

    Applying prescribed ice caps or collars.

    Measuring fluid intake and output.

    Assisting with medications ordinarily self-administered.

     Complete at least 12 hours of in-service training annually.  Handle additional tasks as assigned.  Be available for institutional assignments by VITAS.  Perform specific medical tasks such as changing sterile dressings, irrigating body cavities, colostomies, or wounds, and catheterizing patients.

    Qualifications

    Graduated from an accredited Home Health Aide program

    High School Diploma or GED

    One year prior professional experience

    Driver’s license required

    CPR certification required
