Direct Patient Care
Conducts physical examinations and health histories on patients in a Health Department clinical setting, within the NP scope of practice and recommended guidelines
Reviews all available medical records, interviews patient/parent/guardian to obtain relevant medical, social and family history; obtains full account of current complaint, and systems review; performs full physical examinations using appropriate equipment as per protocol
Interprets lab and other diagnostic tests/results as appropriate to physical findings
Documents findings from clinical exam/labs/diagnostic tests in format utilized by CDC such that documentation adequately supports any prescribed or recommended interventions; uses appropriate VDH forms specific to clinic programs
Develops and records recommended plan of care for patient according to evaluation of all information from history, physical examination and other available information; determines treatment and plan to follow-up, in accordance with established guidelines and standards of practice
Ensures appropriate referrals for effective disease management and prevention/health promotion
Ensures a preventive, population-based public health focus and incorporation of all public health strategies/policies promoted by the district to address community health priorities and to support agency mission
