    At Your Side Home Care in Montgomery is looking for a professional, experienced and personable CNA with leadership and training experience to enhance our team of professional caregivers whom provide in-home care within the Montgomery City of Texas. We are a non-skilled agency who offers our senior clients time and attention with an end goal of providing both our caregiver and our clients day a better day than the day before.

    This certified nursing aide assistance position will be under the direction of the Manager of Care Supervisor. The CNA is assigned to new clients by the Manager of Care Supervisor and will identify and perform services and support for clients as necessary to maintain their personal, physical and emotional comfort. Once desired care and support are identified and provided, the CNA will provide backup care and support to an appropriate companion caregiver employed by the agency and will assure appropriate care and support is provided to all new clients and existing clients through regularly scheduled visits.

    QUALIFICATIONS

    ● Successful completion of a formal certification training program

    ● Be at least eighteen (18) years of age.

    ● Minimum of (5) years of work experience as a CNA in a supervised setting within a health care setting.

    ● Minimum of (2) years of work experience as a leader in supervisory and/or training of caregivers in a health care setting.

    ● Demonstrated ability to read, write, and follow a written Plan of Care/Aide Care Plan.

    ● Good verbal, written, and interpersonal skills.

    ● Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.

    ● Must be familiar with the difference between skilled and non-skilled care (in accordance with state licensure regulations, state practice acts, and Agency policy).

    ● Must be flexible in scheduled hours with an ability to provide care and support a minimum of two weekends and 6 evenings a month (as needed) within a total of 160 hours a month.

    ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

    · Performs and ability to train others in the Texas state standards of personal care activities, including but not limited to:

    | a. Bathing b. Shampooing c. Oral hygiene | d. Dressing Range of motion exercises Assistance in ambulation or exercises | g. Electric Razor | h. Pericare

    · Performs and ability to train others in household services essential to health care at home, including but not limited to:

    | a. Meal preparation/feeding | b. Laundry | c. Light housekeeping

    · Performs and ability to train others in transportation services as authorized by the Agency.

    | a. Doctor’s appointments | b. Other errands

    · Performs and ability to train in the assistance of medication reminders and medication assistance that are ordinarily self-administered (in accordance with state licensure regulations, state practice acts, and Agency policy).

    · Follows Agency policy D180MONT – Medication Assistance. Caregivers are to stay within the Agency’s guidelines and refrains from “dosing”.

    · Performs and ability to train in the reporting of any observed or reported changes in the client's condition and/or needs to the Manager of Care Supervisor. Follows reporting guidelines in the Plan of Care/Aide Care Plan.

    · Performs and ability to train in the documentation of care provided and completes the forms required for the client's records. Submits documentation per Agency requirements—at least weekly.

    · Performs and ability to train in the promotion of personal safety and a safe environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the supervisor.

    · Performs and ability to train in the safe practice in the use of equipment. Notifies supervisor of educational needs.

    · Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.

    · Maintains confidentiality in all aspects of the job.

    · Attends in-service programs to meet compliance requirements. Identifies learning needs to the Agency supervisor.

    · Performs other related duties and responsibilities as deemed necessary.
