Compensation: $21.00-$28.00 (DOE). Differentials $2/hour nights & $2/hour weekend.
Sign-on Bonus: $1,000, paid after 90 days of employment. Shift: Monday-Friday, 8 am-5 pm (full time). Location: Working in the field in patients' homes and facilities throughout Central Oregon.

What You Get To Do:
Assess the patient’s ability to tolerate treatment before beginning care. Assess patient condition aftercare.
Provide personal care as outlined in the plan of care, maintaining patient privacy while demonstrating sensitivity to patient comfort and privacy.
Meet the patient's and self's safety needs by utilizing good body mechanics and proper use of equipment.
Coordinate with a care manager or registered nurse to ensure the plan of care is updated and accurate. Report any changes in the patient’s condition using applicable documentation.
Adhere to written and oral instructions for patient care as indicated by the Registered Nurse on duty.
Assist the patient with all aspects of personal care, ambulation, and positioning as required and provide guidance to the family/caregiver regarding the patient's personal care.
Required Skills & Qualifications:
Valid CNA License.
Ability to pass a background test, drug screen, and TB test.
Graduate of an accredited nursing school.
One to two years of recent medical care experience in an institutional setting is preferred. Ideal candidates will possess 1-2 years of home health or hospice experience.
Current automobile insurance is required. A valid Oregon driver’s license and reliable transportation.
Valid CPR Certification.
Strong communication and interpersonal skills.
Ability to work independently and collaboratively within an interdisciplinary team.
