We are currently seeking a Certified Occupational Therapy Assistant (COTA) to join our team. This position provides Occupational therapy services to patients in the home setting, accepted for care by RVNAhealth as prescribed by the attending physician. The COTA works directly under the direct supervision of a licensed Occupational Therapist and under the overall supervision of the Rehabilitation Manager. This role is a field position that provides therapy services in and out of patient homes and will cover the areas and towns of Central Fairfield County (Ridgefield, Danbury, Wilton).
Key Responsibilities:
Provide ongoing therapy services following an established treatment plan developed by the Registered Occupational Therapist (OT).
Administer treatments related to ADL (activities of daily living), functional mobility, ROM (range of motion), strength, coordination, cognitive/perceptual remediation, functional maintenance, and more.
Assist patients with ambulation, transfers, strengthening techniques, prosthetic training, and adaptive equipment use.
Educate patients, their families, and healthcare staff on the patient’s treatment regimen when necessary.
Document patient care and progress in a timely, accurate manner, meeting regulatory and pay source requirements.
Regularly confer with the OT (at least every 28 days) to review and modify patient care plans as needed.
Participate in discharge planning, including review of the plan of care and summary preparation.
Manage a caseload that meets established productivity standards and maintain communication with the coordinator regarding caseload status.
Attend team meetings, in-services, and educational sessions.
Report changes in patient status to the OT and/or physician as appropriate.
Contribute to patient huddles/case conferences and provide updates as needed.
Adhere to agency and regulatory procedures, including HIPAA, Corporate Compliance, Medicare conditions, and State regulations.
Maintain CPR certification and complete at least 12 hours of in-service training annually.
Qualifications/Requirements:
Graduate of a Certified Occupational Therapy Assistant Program, accredited by the American Occupational Therapy Association or equivalent program.
Current license to practice as an Occupational Therapist Assistant in the State of Connecticut.
At least two (2) years' clinical experience. Preferred Home Health Experience.
Strong Communication Skills.
Reliable transportation and the ability to drive in all types of weather conditions.
