Position: Certified Recovery Peer Advocate
Position Description:
Meeting with new 816 patients
Retention of struggling 816 patients
Discuss aftercare for 816 patients close to completing detox
Engage new 822 patients and orient them to treatment
Outreach to 822 patients struggling with attendance and abstinence
Assist 822 patients with transition/discharging
Attend 822 and 816 case conferences
Essential Duties/Responsibilities:
Developing recovery plans
Raising awareness of existing social and other support services
Modeling coping skills
Assisting with applying for benefits
Accompanying clients to medical appointments
Non-clinical crisis support, especially after periods of hospitalization or incarceration
Working with participants to identify strengths
Linking participants to formal recovery supports
Educating program participants about various modes of recovery
Engaging an individual to consider entering treatment
Co-lead groups
Secondary Duties and Responsibilities: Other appropriate duties as assigned.
