Job Summary

We are looking for a qualified Certified Social Worker (CSW) to join our team! You will play a crucial role in evaluating clients and developing individual treatment plans in collaboration with clients’ physicians.

Responsibilities

Implement standards of care for medical social work services

Participate in clients’ plans of care.

Perform client evaluations and help develop a treatment plan with clients’ physicians

Assess the psychosocial status of the clients as related to their illness

Make follow-up visits to assess and continue the plans of care

Plan interventions based on client's needs and findings

Maintain accurate and up-to-date records

Adhere to all of the Company's operating procedures governing the provision of certified social worker services

Qualifications

Master’s degree in social work

Minimum one year of experience in health care and social work

Current CPR certification

Valid driver’s license

Psych experience is a plus

Possess a active CSW license from Louisiana State Board of Social Work Examiners

Salary: $20.00 - $25.00/hr Anticipated Work Hours: 31-36 hours
