Children’s Behavioral Health (CBH) specialize in providing direct services and case management to children, adolescents and adults with behavioral and medical health needs, as well as those supervised by government subsidized programs, in their homes or through community-based resources. Pathways offers a full spectrum of social service and behavioral health solutions, including Youth & Family Services, Adult Services, and Prevention Services. Within each area, clients have access to a broad range of social and behavioral services to meet their needs.
The CBH Team is seeking a dynamic individual to join our team as a Case Manager providing direct service and related duties to clients, such as: completing authorization packets, ensuring clients are receiving prescribed hours, distributing case loads, participation in team meetings, utilizing excel to update client trackings, coordinate services with agencies, schools, staff and parents.
The Case Manager Position will guide and assist individuals in gaining access to necessary medical rehabilitative and social services. The Case Manager will coordinate and facilitate access to care, and monitor service delivery.

Qualifications
• Bachelor’s degree in a social service field and at least one (1) year experience conducting case management• Reliable transportation is required.
