Position Overview:
A dedicated and compassionate Care Coordinator is sought to join the team. In this hybrid position, the incumbent will primarily focus on providing Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) services to patients with multiple co-morbidities or specific chronic conditions such as diabetes, cardiovascular disease, and COPD. The role involves meticulous attention to program details, benefits, and coordination, and active participation in assisting the team as needed.
Key Responsibilities:
Works diligently on behalf of assigned patients, ensuring their needs are met with empathy and efficiency.
Provides Chronic Care Management and Remote Patient Monitoring services, maintaining a comprehensive understanding of program requirements and benefits.
Gathers accurate information and documents patient interactions in the Electronic Medical Record (EMR) system.
Possesses certification as a Nursing Assistant (CNA), Medical Assistant (MA), or MedTech.
Demonstrates passion for patient care, with exceptional empathy and compassion.
Maintains a HIPAA-compliant workspace to safeguard patient privacy during interactions.
Possesses a reliable computer with fast internet connectivity and necessary peripherals for efficient remote work.
Meets or exceeds outbound connect rates, compliance standards, caseload management, and billable time requirements.
Performs data entry and documentation of patient interactions in designated systems.
Adheres to regulatory guidelines and company policies, ensuring compliance.
Assists with the delivery of Remote Patient Monitoring devices as needed.
Handles pre-authorizations, referrals, prescription refill issues, and medication reconciliation tasks.
Coordinates durable medical equipment (DME) services with vendors and reviews hospital records for care planning purposes.
Collaborates with outside services and Primary Care Physicians (PCPs) for comprehensive patient care.
Provides patient education on prescriptions, disease processes, and lifestyle recommendations.
Addresses patient inquiries, triage notes, and requests for callbacks promptly and efficiently.
Collaborates with healthcare providers to manage medication changes and address clinical concerns effectively.
Qualifications:
Possesses certification as a Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or Med Tech, OR designated as a Medical Assistant (MA) by a Licensed Physician affiliated with D&G Medical Associates, PLLC.
Strong organizational skills with meticulous attention to detail.
Excellent communication and interpersonal skills.
Ability to work independently and collaboratively within a team.
Proficiency in using Electronic Medical Record (EMR) systems and other relevant software applications.
