    Position Overview: The Client Care Coordinator is responsible for managing and overseeing the daily operations related to client care within a non-medical home care environment. This role involves developing and maintaining strong relationships with clients and caregivers, coordinating client services, ensuring the quality of care, and acting as the main point of contact for clients, families, and caregivers. The Client Care Coordinator ensures that clients receive the highest level of care and that all services are delivered in accordance with company standards and policies.

    Responsibilities:

    Client Intake & Assessment:

    Conduct initial intake assessment with potential clients and their families.

    Assess clients’ care needs and develop personalized care plans.

    Maintain regular communication with clients to assess ongoing needs and adjust care plans as necessary.

    Caregiver Assignment & Scheduling:

    Match clients with qualified care providers, ensuring compatibility and continuity of care.

    Coordinate and schedule caregivers for home visits.

    Monitor and supervise care providers to ensure adherence to care plans and delivery of high-quality services.

    Ensure that caregivers are informed and prepared to meet each client’s specific care needs.

    Client Relations:

    Act as the primary point of contact for clients and their families, addressing any concerns or questions.

    Perform routine quality checks, including regular phone calls and home visits, to ensure client satisfaction. Document supervisory visits accordingly,

    Provide timely follow-up on client inquiries and concerns to ensure a high level of customer service.

    Caregiver Support & Management:

    Provide ongoing support to caregivers, offering guidance and resolving scheduling or performance issues.

    Work with the HR department to ensure caregivers meet all compliance and certification requirements.

    Assist in training and onboarding new caregivers as needed.

    Documentation & Compliance:

    Maintain detailed client records, including care plans, progress notes, and communication logs.

    Ensure all documentation complies with company policies, local and federal regulations, and privacy laws.

    Monitor and report any changes in clients’ health status or care needs.

    Coordination of Services:

    Collaborate with other care providers, such as healthcare professionals including home health and hospice, to ensure a holistic approach to care.

    Arrange for additional services, such as transportation, companionship, or meal delivery, as needed.

    Quality Assurance & Improvement:

    Develop and implement quality control processes to ensure excellent client care.

    Conduct regular quality assurance assessments to evaluate the effectiveness and appropriateness of care services.

    Address any issues or deficiencies identified through quality assurance assessments promptly and effectively.

    Participate in ongoing training and development to stay current with best practices in home care services.

    Qualifications:

    Education: High school diploma or equivalent required; Bachelor's degree in healthcare, social work, or a related field preferred.

    Experience: Previous experience in home care, healthcare, or customer service, particularly in a coordinator or supervisory role.

    Skills:

    Strong communication and interpersonal skills.

    Excellent organizational and multitasking abilities.

    Problem-solving skills and the ability to work well under pressure.

    Proficiency with scheduling software and basic office technology.

    Familiarity with local and federal regulations regarding home care services.

    Ability to work flexible hours, including evenings or weekends, as needed.

    Valid driver’s license and reliable transportation for client visits.
