    Visiting Angels of Raynham is looking for a Client Care Coordinator - Scheduler Schedule: M-F 9:00 AM-5:00 PM Location: Raynham MA Employment status: Full time

    Job Description

    Client Care Coordinator Scheduler duties include, but are not limited to:

    Answer the telephone and greet the public

    Respond to clients’ inquiries, and conduct clients’ assessments

    Schedule caregivers-clients based on caregiver’s availability and client needs

    Review cost of care with clients, and complete service agreement and initial paperwork

    Develop plan of care in accordance with client needs and update plans as warranted

    Facilitate initial client and caregiver meetings

    Independently address clients’ issues, concerns and complaints

    Work with all clients and maintain positive relationships with referral sources including the Veterans Administration, ASAP (Aging Services Access Point) such as Old Colony Elder Services and Bristol Elder Services.

    Participate in agency’s referral management system by compiling the database, managing leads and communicate with referral sources

    Be dependable; arrive on time and complete assigned tasks with minimal supervision.

    Advertise and help recruit direct care staff such as HHA’s and CNA’s etc.

    Conduct phone-screening interviews, process applications, check references and schedule interviews for potential employees

    Help conduct new caregiver staff orientation.

    Maintain and appropriately file documentation on all current and past clients.

    Participate in annual evaluations of caregiver staff.

    Maintain absolute confidentiality of all information pertaining to employees and clients

    Be knowledgeable of the agency services and clients base and communicate information on agency’s competition.

    Reviewing schedule accuracy for payroll and billing

    Perform other clerical functions as deemed appropriate by the management team.

    REQUIRED JOB KNOWLEDGE AND SKILLS:

    High school diploma or equivalent

    Customer service and good phone etiquette

    Proficiency with Microsoft Office (Word, Excel and Outlook) applications

    Ability to listen and communicate clearly, fluently, and diplomatically - both orally and in writing

    Present a well-groomed image that reflects the professionalism of the business.

    Ability to plan, organize, prioritize, and accurately complete work activities within specific deadlines while appropriately managing interruptions

    Work independently with a minimum direction and/or supervision after training.

    Patience and the ability to handle pressure and deadlines

    Ability to work and thrive in a fast-paced environment and handle pressure and deadlines

    Knowledge of healthcare industry-related software such as WellSky and ClearCare a plus

    Familiarity with ASAP (Aging Services Access Point) a plus.

    Ability to lawfully work in the U.S.

    PHYSICAL/ENVIRONMENTAL DEMANDS:

    Combination of sitting, standing, bending, reaching, stretching, stooping, walking, driving, climbing stairs and moving during working hours

    Must be able to lift at least 50 lbs., if working with clients

    Must be able to properly operate office equipment

    Successful completion of background testing, CORI, drug and alcohol testing etc
